Math Beta Testers Feedback Form

This form is a chance for you to give feedback: what worked, what didn't, errors or typos, etc. Together, we are confident that we can create a math program that will work for many different learning styles and situations! (You will need to complete this form once for each step and/or product. It is ideal to compile all of your submissions into one entry for that book, but if things come up later, please don't hesitate to fill this out again as we want to hear about every detail!)

Please provide as many details as possible. What level, page number... as much as you can give us. We will have a place at the end of this form for you to attach any screenshots you may have. If you have the ability, you can also circle or point to problems on the images you provide as well.

If you have any images of your student doing the math, please feel free to upload photos of math in action for a chance to win a $25 Amazon gift card! This will be a draw available at the end of each unit and you are entered each form you fill out/picture you add/testimonial you give. By submitting these photos/testimonials, you give us permission to use them for marketing.
